A) First , Go to the RMA Setting -> Basic Setting -> Predefined Refund Reason -> click on "add more" button if you want to add more Refund reasons and Go to the RMA Setting -> Basic Setting -> Predefined Exchange Reason -> click on "add more" button if you want to add more Exchange reasons .
